New application for Renewal of Mandatory (HMO) Licence

This application form is for the year 6 renewal of HMO properties licenced under the mandatory licensing scheme. To complete this application will take around 45 minutes, You should ensure you have the following information to hand:

  • Evidence of business/permanent residential address of the licence holder
  • Evidence of identity of proposed licence holder
  • Disclosure and Barring Services (DBS) form
  • Copy of management agreement with owner - If applicable
  • Copy of current tenancy agreement
  • Annual maintenance record for automatic fire detection system
  • Emergency lighting system test certificate - If applicable
  • Evidence of identity of proposed licence holder
  • Service contract for alarm and fire systems
  • Gas Safe registered commissioning and annual gas safety inspection certificate
  • Electrical inspection certificate (issued within the last 5 years)

Please note that it is a criminal offence to make a false statement in an application for an HMO licence or fail to comply with any condition of the licence.
